Other providers expect this as a cohesive form. Even your own page describing a CSR clearly explains the form that we simply cannot generate using the portal or even by contacting support. https://support.ringcentral.com/article-v2/Submitting-your-Porting-order.html?brand=RingCentral&product=RingEX&language=en_US&pills-nav=acronyms
If all of the information is available in the portal and is "simple" for the customer to put together, then it should be "simple" for RingCentral to provide a combined form.
